Due to how the Environment class is used through the codebase,
there are a lot of includes referencing either env.h or env-inl.h.
This can cause that when any development touches those libraries,
a lot of files have to be recompiled.
This commit attempts to change those includes by forward declarations
when possible to mitigate the issue.

TLSWrap::DoWrite() now concatenates data chunks and makes a single
call to SSL_write(). Grouping data into a single segment:
- reduces network overhead: by factors of even 2 or 3 in usages
like `http2` or `form-data`
- improves security: segment lengths can reveal lots of info, i.e.
with `form-data`, how many fields are sent and the approximate length
of every individual field and its headers
- reduces encryption overhead: a quick benchmark showed a ~30% CPU time
decrease for an extreme case, see

Exposes SSL_CTX_set_keylog_callback in the form of a `keylog` event
that is emitted on clients and servers. This enables easy debugging
of TLS connections with i.e. Wireshark, which is a long-requested
feature.

Enable the same trace output that the OpenSSL s_client and s_server
support with their `-trace` option. This is invaluable when debugging
reports of TLS bugs as well as when debugging the internal TLS
implementation.

This introduces TLS1.3 support and makes it the default max protocol,
but also supports CLI/NODE_OPTIONS switches to disable it if necessary.
TLS1.3 is a major update to the TLS protocol, with many security
enhancements. It should be preferred over TLS1.2 whenever possible.
TLS1.3 is different enough that even though the OpenSSL APIs are
technically API/ABI compatible, that when TLS1.3 is negotiated, the
timing of protocol records and of callbacks broke assumptions hard-coded
into the 'tls' module.
This change introduces no API incompatibilities when TLS1.2 is
negotiated. It is the intention that it be backported to current and LTS
release lines with the default maximum TLS protocol reset to 'TLSv1.2'.
This will allow users of those lines to explicitly enable TLS1.3 if they
want.
API incompatibilities between TLS1.2 and TLS1.3 are:
- Renegotiation is not supported by TLS1.3 protocol, attempts to call
`.renegotiate()` will always fail.
- Compiling against a system OpenSSL lower than 1.1.1 is no longer
supported (OpenSSL-1.1.0 used to be supported with configure flags).
- Variations of `conn.write('data'); conn.destroy()` have undefined
behaviour according to the streams API. They may or may not send the
'data', and may or may not cause a ERR_STREAM_DESTROYED error to be
emitted. This has always been true, but conditions under which the write
suceeds is slightly but observably different when TLS1.3 is negotiated
vs when TLS1.2 or below is negotiated.
- If TLS1.3 is negotiated, and a server calls `conn.end()` in its
'secureConnection' listener without any data being written, the client
will not receive session tickets (no 'session' events will be emitted,
and `conn.getSession()` will never return a resumable session).
- The return value of `conn.getSession()` API may not return a resumable
session if called right after the handshake. The effect will be that
clients using the legacy `getSession()` API will resume sessions if
TLS1.2 is negotiated, but will do full handshakes if TLS1.3 is

Renamed some internal C++ methods and properties for consistency, and
commented SSL I/O.
- Rename waiting_new_session_ after is_waiting_new_session(), instead of
using reverse naming (new_session_wait_), and change "waiting" to
"awaiting".
- Make TLSWrap::ClearIn() return void, the value is never used.
- Fix a getTicketKeys() cut-n-paste error. Since it doesn't use the
arguments, remove them from the js wrapper.
- Remove call of setTicketKeys(getTicketKeys()), its a no-op.

This patch:
- Refactors the `MemoryRetainer` API so that the impementer no longer
calls `TrackThis()` that sets the size of node on the top of the
stack, which may be hard to understand. Instead now they implements
`SelfSize()` to provide their self sizes. Also documents
the API in the header.
- Refactors `MemoryTracker` so it calls `MemoryInfoName()` and
`SelfSize()` of `MemoryRetainer` to retrieve info about them, and
separate `node_names` and `edge_names` so the edges can be properly
named with reference names and the nodes can be named with class
names. (Previously the nodes are named with reference names while the
edges are all indexed and appear as array elements).
- Adds `SET_MEMORY_INFO_NAME()`, `SET_SELF_SIZE()` and
`SET_NO_MEMORY_INFO()` convenience macros
- Fixes a few `MemoryInfo` calls in some `MemoryRetainers` to track
their references properly.
- Refactors the heapdump tests to check both node names and edge names,
distinguishing between wrapped JS nodes (without prefixes)
and embedder wrappers (prefixed with `Node / `).

- Use camel case names for memory retainers inherited from AsyncWrap
instead of their provider names (which are all in upper case)
- Assign class names to wraps so that they appear in the heap snapshot
as nodes with class names as node names. Previously some nodes are
named with reference names, which are supposed to be edge names
instead.

The root cause is that `req_wrap` is created in `StreamBase::Write`
and passed to `TLSWrap::DoWrite`. In the TLS case the object gets
disposed and replaced with a new instance, but the caller's pointer is
never updated. When the `StreamBase::Write` method returns, it returns
a pointer to the freed object to the caller. In some cases when the
object memory has already been reused an assert is hit in
`WriteWrap::SetAllocatedStorage` because the pointer is non-null.

Encapsulate stream requests more:
- `WriteWrap` and `ShutdownWrap` classes are now tailored to the
streams on which they are used. In particular, for most streams
these are now plain `AsyncWrap`s and do not carry the overhead
of unused libuv request data.
- Provide generic `Write()` and `Shutdown()` methods that wrap
around the actual implementations, and make *usage* of streams
easier, rather than implementing; for example, wrap objects
don’t need to be provided by callers anymore.
- Use `EmitAfterWrite()` and `EmitAfterShutdown()` handlers to
call the corresponding JS handlers, rather than always trying
to call them. This makes usage of streams by other C++ code
easier and leaner.
Also fix up some tests that were previously not actually testing
asynchronicity when the comments indicated that they would.

Instead of setting individual callbacks on streams and tracking
stream ownership through a boolean `consume_` flag, always have
one specific listener object in charge of a stream, and call
methods on that object rather than generic C-style callbacks.

The TLS implementation previously kept a separate buffer for
incoming pieces of data, into which buffers were copied
before they were up for writing.
This removes this buffer, and replaces it with a simple list
of `uv_buf_t`s:
- The previous implementation copied all incoming data into
that buffer, both allocating new storage and wasting time
with copy operations. Node’s streams/net implementation
already has to make sure that the allocated memory stays
fresh until the write is finished, since that is what
libuv streams rely on anyway.
- The fact that a separate kind of buffer, `crypto::NodeBIO`
was used, was confusing: These `BIO` instances are
only used to communicate with openssl’s streams system
otherwise, whereas this one was purely for internal
memory management.
- The name `clear_in_` was not very helpful.

The TLS implementation previously had two separate queues for
WriteWrap instances, one for currently active and one for
finishing writes (i.e. where the encrypted output is being written
to the underlying socket).
However, the streams implementation in Node doesn’t allow for
more than one write to be active at a time; effectively,
the only possible states were that:
- No write was active.
- The first write queue had one item, the second one was empty.
- Only the second write queue had one item, the first one was empty.
To reduce overhead and implementation complexity, remove these
queues, and instead store a single `WriteWrap` pointer and
keep track of whether its write callback should be called
on the next invocation of `InvokeQueued()` or not
(which is what being in the second queue previously effected).

Currently, writeQueueSize is never used in C++ and barely used
within JS. Instead of constantly updating the value on the JS
object, create a getter that will retrieve the most up-to-date
value from C++.
For the vast majority of cases though, create a new prop on
Socket.prototype[kLastWriteQueueSize] using a Symbol. Use this
to track the current write size, entirely in JS land.

Instead of having per-request callbacks, always call a callback
on the `StreamBase` instance itself for `WriteWrap` and `ShutdownWrap`.
This makes `WriteWrap` cleanup consistent for all stream classes,
since the after-write callback is always the same now.
If special handling is needed for writes that happen to a sub-class,
`AfterWrite` can be overridden by that class, rather than that
class providing its own callback (e.g. updating the write
queue size for libuv streams).
If special handling is needed for writes that happen on another
stream instance, the existing `after_write_cb()` callback
is used for that (e.g. custom code after writing to the
transport from a TLS stream).
As a nice bonus, this also makes `WriteWrap` and `ShutdownWrap`
instances slightly smaller.

Add updateWriteQueueSize which updates and returns queue size
(net & tls). Make _onTimeout check whether an active write
is ongoing and if so, call _unrefTimer rather than emitting
a timeout event.
Add http & https test that checks whether long-lasting (but
active) writes timeout or can finish writing as expected.

crypto namespace but simply in the node namespace. Not sure if this was
intentional or not, but I think it would make sense to move them to be

The TLSWrap constructor is passed a StreamBase* which it stores as
TLSWrap::stream_, and is used to receive/send data along the pipeline
(e.g. tls -> tcp). Problem is the lifetime of the instance that stream_
points to is independent of the lifetime of the TLSWrap instance. So
it's possible for stream_ to be delete'd while the TLSWrap instance is
still alive, allowing potential access to a then invalid pointer.
Fix by having the StreamBase destructor null out TLSWrap::stream_;
allowing all TLSWrap methods that rely on stream_ to do a check to see
if it's available.
While the test provided is fixed by this commit, it was also previously
fixed by 478fabf. Regardless, leave the test in for better testing.

TLSWrap object keeps a pointer reference to the underlying
TCPWrap object. This TCPWrap object could be closed and deleted
by the event-loop which leaves us with a dangling pointer.
So the TLSWrap object needs to track the "close" event on the
TCPWrap object.

`WrapperInfo` casts pointer in JS object's internal field to
`AsyncWrap`. This approach fails miserably for `TLSWrap` because it was
inhereted from the `StreamBase` first, creating different kind of
`vtable` for the whole class.
Reorder parent classes to put `AsyncWrap` first.

1e066e4a was done incorrectly and has overwritten an important change
in: c17449df. Using bigger output buffer increases performance in 3-4
times.

Re-add the wrapper class id to AsyncWrap instances so they can be
tracked directly in a heapdump.
Previously the class id was given without setting the heap dump wrapper
class info provider. Causing a segfault when a heapdump was taken. This
has been added, and the label_ set to the given provider name so each
instance can be identified.
The id will not be set of the passed object has no internal field count.
As the class pointer cannot be retrieved from the object.
In order to properly report the allocated size of each class, the new
pure virtual method self_size() has been introduces.

Do not enable ClientHello parser for async SNI/OCSP. Use new
OpenSSL-1.0.2's API `SSL_set_cert_cb` to pause the handshake process and
load the cert/OCSP response asynchronously. Hopefuly this will make
whole async SNI/OCSP process much faster and will eventually let us
remove the ClientHello parser itself (which is currently used only for
async session, see #1462 for the discussion of removing it).
NOTE: Ported our code to `SSL_CTX_add1_chain_cert` to use
`SSL_CTX_get0_chain_certs` in `CertCbDone`. Test provided for this
feature.
