Node codebase has evolved a lot in the more than 10 years of its
existence. As more features (and code) have been added, changed,
removed, it's sometimes hard to keep track of what gets used and what
not.
This commits attempts to clean some of those potentially left-over
headers using suggestions from include-what-you-use

`v8::Global` is essentially a nicer variant of `node::Persistent` that,
in addition to reset-on-destroy, also implements move semantics.
This commit makes the necessary replacements, removes
`node::Persistent` and (now-)unnecessary inclusions of the
`node_persistent.h` header, and makes some of the functions that
take Persistents as arguments more generic so that they work with all
`v8::PersistentBase` flavours.

This PR updates the current `--experimental-modules` implementation
based on the work of the modules team and reflects Phase 2 of our
new modules plan.
The largest differences from the current implementation include
* `packge.type` which can be either `module` or `commonjs`
- `type: "commonjs"`:
- `.js` is parsed as commonjs
- default for entry point without an extension is commonjs
- `type: "module"`:
- `.js` is parsed as esm
- does not support loading JSON or Native Module by default
- default for entry point without an extension is esm
* `--entry-type=[mode]`
- allows you set the type on entry point.
* A new file extension `.cjs`.
- this is specifically to support importing commonjs in the
`module` mode.
- this is only in the esm loader, the commonjs loader remains
untouched, but the extension will work in the old loader if you use
the full file path.
* `--es-module-specifier-resolution=[type]`
- options are `explicit` (default) and `node`
- by default our loader will not allow for optional extensions in
the import, the path for a module must include the extension if
there is one
- by default our loader will not allow for importing directories that
have an index file
- developers can use `--es-module-specifier-resolution=node` to
enable the commonjs specifier resolution algorithm
- This is not a “feature” but rather an implementation for
experimentation. It is expected to change before the flag is
removed
* `--experimental-json-loader`
- the only way to import json when `"type": "module"`
- when enable all `import 'thing.json'` will go through the
experimental loader independent of mode
- based on https://github.com/whatwg/html/issues/4315
* You can use `package.main` to set an entry point for a module
- the file extensions used in main will be resolved based on the
`type` of the module

This patch:
- Refactors the `MemoryRetainer` API so that the impementer no longer
calls `TrackThis()` that sets the size of node on the top of the
stack, which may be hard to understand. Instead now they implements
`SelfSize()` to provide their self sizes. Also documents
the API in the header.
- Refactors `MemoryTracker` so it calls `MemoryInfoName()` and
`SelfSize()` of `MemoryRetainer` to retrieve info about them, and
separate `node_names` and `edge_names` so the edges can be properly
named with reference names and the nodes can be named with class
names. (Previously the nodes are named with reference names while the
edges are all indexed and appear as array elements).
- Adds `SET_MEMORY_INFO_NAME()`, `SET_SELF_SIZE()` and
`SET_NO_MEMORY_INFO()` convenience macros
- Fixes a few `MemoryInfo` calls in some `MemoryRetainers` to track
their references properly.
- Refactors the heapdump tests to check both node names and edge names,
distinguishing between wrapped JS nodes (without prefixes)
and embedder wrappers (prefixed with `Node / `).

- Use camel case names for memory retainers inherited from AsyncWrap
instead of their provider names (which are all in upper case)
- Assign class names to wraps so that they appear in the heap snapshot
as nodes with class names as node names. Previously some nodes are
named with reference names, which are supposed to be edge names
instead.

Replace v8::Persistent with node::Persistent, a specialization that
resets the persistent handle on destruction. Prevents accidental
resource leaks when forgetting to call .Reset() manually.
I'm fairly confident this commit fixes a number of resource leaks that
have gone undiagnosed so far.

This adds support for ensuring that the top-level main into Node is
supported loading when it has no extension for backwards-compat with
NodeJS bin workflows.
In addition package.json caching is implemented in the module lookup
process.

This is an initial implementation to support dynamic import in
both scripts and modules. It's off by default since support for
dynamic import is still flagged in V8. Without setting the V8 flag,
this code won't be executed.
This initial version does not support importing into vm contexts.

Various improvements on readability, performance and
conformity to the Node core coding style in the ESM loader C++ code:
- `isolate` for the `Isolate*`, `context` for the `Local<Context>`
- Less reliance on `auto` where it’s unnecessary/increases cognitive
overhead
- Shorter paths to failure via `.ToLocal()` & co
- Do not keep pending exceptions around e.g. for failed `JSON` parsing
- Use `Maybe` types to get explicit error status forwarding
- Remove an unnecessary handle scope
- Add `nullptr` checks for unwrapped host objects
- Remove unused code
- Use `CamelCase` for function names
- Use `const Foo&` instead of copying values whenever possible
- Pass along the `Environment*` explicitly

- Move `module_map` to `Environment` instead of having it be global
state

This follows the EPS an allows the node CLI to have ESM as an entry point.
`node ./example.mjs`. A newer V8 is needed for `import()` so that is not
included. `import.meta` is still in specification stage so that also is not
included.
