For the one remaining usecase inside of `lib/vm.js`, define a Symbol at
the top of the file.

The agent code is supposed to manage multiple writers/clients.
Adding the default writer into the mix breaks that encapsulation.
Instead, manage default options through a separate "virtual"
default client handle, and keep the file writer management
all to the actual users.

Refactor Timers to behave more similarly to Immediates by having
a single uv_timer_t handle which is stored on the Environment.
No longer expose timers in a public binding and instead make
it part of the internalBinding.

Scrypt is a password-based key derivation function that is designed to
be expensive both computationally and memory-wise in order to make
brute-force attacks unrewarding.
OpenSSL has had support for the scrypt algorithm since v1.1.0. Add a
Node.js API modeled after `crypto.pbkdf2()` and `crypto.pbkdf2Sync()`.
Changes:
* Introduce helpers for copying buffers, collecting openssl errors, etc.
* Add new infrastructure for offloading crypto to a worker thread.
* Add a `AsyncWrap` JS class to simplify pbkdf2(), randomBytes() and
scrypt().

Logic is added to the `MessagePort` mechanism that
attaches hidden objects to those instances when they are transferred
that track their lifetime and maintain a reference count, to make
sure that memory is freed at the appropriate times.

Implement `MessagePort` and `MessageChannel` along the lines of
the DOM classes of the same names. `MessagePort`s initially
support transferring only `ArrayBuffer`s.

Implement utilities for easier debugging of Node.js core code,
inspired by the HTTP/2 debugging code. Debugging is, however,
implemented at runtime rather than at compile time, controlled
through a new `NODE_DEBUG_NATIVE=categories` environment variable.
The runtime overhead in the debugging-disabled case amounts to
1 well-cachable one-byte read per debug call.

Workers cannot shut down while requests are open, so keep a counter
that is increased whenever libuv requests are made and decreased
whenever their callback is called.
This also applies to other embedders, who may want to shut down
an `Environment` instance early.

Previously, handles would not be closed when the current `Environment`
stopped, which is acceptable in a single-`Environment`-per-process
situation, but would otherwise create memory and file descriptor
leaks.
Also, introduce a generic way to close handles via the
`Environment::CloseHandle()` function, which automatically keeps
track of whether a close callback has been called yet or not.

This adds pairs of methods to the `Environment` class and to public APIs
which can add and remove cleanup handlers.
Unlike `AtExit`, this API targets addon developers rather than
embedders, giving them (and Node’s internals) the ability to register
per-`Environment` cleanup work.
We may want to replace `AtExit` with this API at some point.

Removes the requirement to use `--trace-events-enabled` to enable
trace events. Tracing is enabled automatically if there are any
enabled categories.
Adds a new `trace_events` module with an API for enabling/disabling
trace events at runtime without a command line flag.
```js
const trace_events = require('trace_events');
const categories = [ 'node.perf', 'node.async_hooks' ];
const tracing = trace_events.createTracing({ categories });
tracing.enable();
// do stuff
tracing.disable();
```
Multiple `Tracing` objects may exist and be enabled at any point
in time. The enabled trace event categories is the union of all
enabled `Tracing` objects and the `--trace-event-categories`
flag.

- Pass kFsStatsFieldsLength between JS and C++ instead of using the
magic number 14
- Pass the global stats array to the completion callback of
asynchronous FSReqWrap similar to how the stats arrays are passed
to the FSReqPromise resolvers
- Abstract the stats converter and take an offset to compute the
old stats in fs.watchFile
- Use templates in node::FillStatsArray and FSReqPromise in preparation
for BigInt intergration
- Put the global stat array filler in node_internals.h because it is
shared by node_file.cc and node_stat_watcher.cc

- Merge `domain` and `opaque` storage in URL parser:
This just simplifies the code a bit, having multiple fields
in an union with the same type is usually just overhead.
- Add move variant of `URLHost::ToString()`:
This helps avoid unnecessary string copy operations, especially
since we control the lifetime of `URLHost` objects pretty well.
- Use const refs in node_url.cc where appropriate
- Remove or reduce overly generous `.reserve()` calls:
These would otherwise keep a lot of unused memory lying around.
- Return return values instead of unnecessary pointer arguments
- Use more common/expressive variable names
- Avoid macro use, reduce number of unnecessary JS strings:
There’s no reason for `GET`, `GET_AND_SET` and `UTF8STRING` to be
macros. Also, `GET` would previously create a JS string instance
for each single call, even though the strings it was called
with were compile-time constants.
- Avoid unnecessary JS casts when the type of a value is known
- Avoid (commonly unnecessary) copy for whitespace stripping

Move tracking of `socket.bytesWritten` to C++ land.
This makes it easier to provide this functionality for all
`StreamBase` instances, and in particular should keep working
when they have been 'consumed' in C++ in some way (e.g. for
the network sockets that are underlying to TLS or HTTP2 streams).
Also, this parallels `socket.bytesRead` a lot more now.

NPN in 2016 and 2017 respectively to no ill effect.

- Make the HandleCleanup a struct, and make the queue a
std::list, iterate over it in CleanupHandles() and clear
it after that.
- Put the handle cleanup registration into a method and
document that they will not be called in the one environemt
per process setup.

The `StreamBase` interface changed, so that `OnStreamRead()`
and `OnStreamAlloc()` are not guaranteed to be emitted in the
same tick any more.
This means that, while it isn’t causing any trouble right now,
we should not assume that it’s safe to return a static buffer
in the HTTP parser’s `OnStreamAlloc()` method.

This allows V8 to avoid preparing a execution context
for the constructor, to give a (kinda) small but noticeable
perf gain.
Benchmarks (only this commit):

This enables accessing files using a more standard pattern.
Once some more refactoring has been performed on the other existing
`StreamBase` streams, this could also be used to implement `fs`
streams in a more standard manner.
